Statements should be filled out by the objector in duplicate, and submitted to the Student Registry Office in the case of an undergraduate student, to the Secretary of the Department of affiliation in the case of a worker.

Of the two copies one will be retained while the other, properly stamped and/or countersigned, will be returned to the objector.
